There is a simple 3 step process to ensure that we are able to meet your travel needs:
In order to give you the best advice and arrange the correct vaccines, we need to know a little bit more about your planned trip. If you can review and complete the forms below and send them before your appointment, we can make sure we have everything ready for you. Alternatively we can help you fill out the forms during a consultation, but this may mean you incur a consultation fee. If you send over the forms before your appointment will be free, you’ll only be charged for the recommended vaccines and treatments.
Step 1
Please complete the ‘Travel Clinic Risk Assessment Form‘. You will also need to complete a ‘Yellow Fever Risk Assessment Form’ if you believe you will require this vaccination.
Step 2
Please email the completed forms to travelclinic@aggarwalgroup.co.uk.
Step 3
The clinic will then contact you, usually within 2 working days, to discuss the recommended treatment programme and to arrange a convenient appointment.
Alternatively if for any reason you are unable to follow the above process, please contact us.
